Rebate Redemption Instructions: (Please follow these instructions. No exceptions will be made.)
1. Mail the completed rebate certificate including model and serial number to the address below.
2. Include a legible copy of your sales receipt showing model number, purchase date, and all charges paid.
 3. Eligible models must be purchased between August 27, 2004 and September 6, 2004.
4. Envelope must be postmarked by September 20, 2004. Must take delivery by September 20, 2004.
5. Allow 12 weeks from the time we receive all required information for delivery of your rebate check.
This rebate offer is limited to one rebate, per product line, per family, per household. Multiple sales to apartments, condominiums, subdivisions, wholesale sales, etc. do not qualify. Void where prohibited, taxed, or restricted by law. Missing, incomplete, or incorrect information delays rebate processing. Please complete all fields requesting information. Please DO NOT USE tape, staples, paper clips, etc. when  mailing information. Model purchased must be checked and you must provide the serial number of that model on the form in order to be eligible. If you do not receive your rebate after 12 weeks please call 1-800-249-8376 between 9 & 5 PM EST. The consumer is responsible for appropriate state sales tax. The consumer is solely responsible for lost, mutilated, or misdirected mail. If you receive a letter from us requesting more information, all responses must be made within 4 weeks of our request. By submitting this claim you hereby accept the above stated terms and conditions. Fraudulent submissions may result in federal prosecution. (NMG2114)
